Solution:

Built 'Sofía,' a published inbound Vapi assistant configured with Deepgram Nova 3 Multilingual transcription, GPT-4o Mini Cluster, and Vapi's Paige voice. The agent opens in Spanish ('Hola, soy Sofía de Clínica Demo'), detects the caller's language early per its prompt rules, and continues the entire conversation in English or Spanish as the AI treatment coordinator, with a templated {{clinic}} variable making the build reusable across practices, and tone guidelines enforcing natural, non-robotic phrasing with a never-guess clarification rule.

Result:

Spanish and English callers now get a fluent same-language treatment coordinator on every inbound call, running at ~$0.09/min with ~840ms latency.
